Job Summary

Under the direction of the immediate Supervisor, or designate, and in collaboration as member of a multidisciplinary team, performs assessments, plans and provides care and performs nursing procedures to adults with a developmental disability. The RN/ LPN responds to emergency situations to safeguard clients, visitors and staff. As required, s/he provides support and direction to co-workers including unlicensed staff involved in direct client care.

Duties and responsibilities

Implements a plan of care which is based on collected data and current care standards. Performs nursing care and procedures and evaluates outcomes.

Assesses client’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.

Provide care according to the established care plan ensuring that, on an ongoing basis, the emotional, spiritual, physical, comfort and safety of clients are met to the greatest possible extent

Preceptors, orients, mentors students or new staff, or any caregiver as required.

Provide nursing care in community, and/or home settings that could occur at any time within a 24/7 period.

Perform non-nurse tasks as required, depending upon the Client’s education or developmental needs, structuring activities to support optimal physical and emotional development. (e.g. grocery shopping, laundry)

Develop and maintain positive relationships with the Client, Family, Agency Staff, administrative employees and other service

Participate in meetings as required for the purpose of discussing issues relevant to the Client’s Direct Nursing Care.

Processes and implement physician orders, identifying and seeking clarification as necessary, as per policies and guidelines.

Travel with Client as required, to provide nursing care as required. (E.g. public transit, private transit, ambulance, etc.)

Perform routine nursing duties and perform specialized nursing procedures for which recognized training and current competency can be demonstrated.

Perform routine household tasks as required, to support the Client in their home, including all tasks to maintain a household in the community. (E.g. sweeping, vacuuming, watering, light maintenance, shoveling snow if required.)

Participate in ongoing internal and/or external continuing education activities to meet professional continuing competency requirements.

Initiates, maintains and concludes a professional relationship with clients and colleagues using channels to communicate and resolve conflict.

Participate in proactive Health & Safety activities while performing all duties.


Graduate of a recognized nursing program with a current, full licensure

Basic Life Support, Level “C/HPC”

Demonstrate knowledge and competencies to work at full scope of practice, including administering medications.

Physical ability to perform the duties of the position.

Ability to remain calm and to act appropriately in stressful and emergency situations.

Additional Information:

This position requires Union membership.

Hours: Full time, Part time and casual. All shifts

Wage rate: As per College of Nursing grid plus benefits (dental, extended health, LTD, Municipal Pension Plan)
